This is a 2006 documentary about the deadliest plane crash, which occurred on March 27, 1977 on the island of Tenerife in the Canary Islands.
The crash killed 583 people, making it the deadliest accident in aviation history.
The investigation revealed that the primary cause of the accident was the captain of the KLM flight taking off without clearance from Air Traffic Control.
The flight was one of Captain Veldhuyzen van Zanten's first after spending six months training new pilots on a flight simulator, where he had been in charge of everything, hence having been away from the real world of flying for an extended period.
The flight engineer and the first officer noticed that something was wrong but didn't challenge Veldhuyzen van Zanten further because the captain was not only senior in rank, but also one of the most respected pilots working for the airline.
Nowadays, less experienced flight crew members are encouraged to challenge their captains when they believe something is not correct, and captains are instructed to listen to their crew and evaluate all decisions in light of crew concerns.
The old sying goes: nobody is perfect.
This concept is known today as Crew Resource Management, in which training is now mandatory for all airline pilots.
